Surgery Death Blamed On ‘Cocktail Of Circumstances’

A coronial inquest has determined the death of a 68-year-old Western Australian woman during a gall bladder procedure was the result of human error, system failure and bad luck.

The woman underwent surgery to take out a gall bladder that had already been removed a decade earlier at the same hospital.
